Summer Internship Forms
CAREER GATEWAY INTERNSHIP APPLICATION PROCESS FOR ACCEPTED STUDENTS
 
After being accepted for a summer internship through Career Gateway, students must prepare the following documents and submit them to the relevant person in the midwifery department.
 
*Document confirming acceptance for a summer internship from the Presidency's Human Resources Office Career Gateway
 
*Three copies of the internship agreement must be signed with a wet signature. One copy will remain at the institution where the internship will be conducted (employer), one at the faculty, and one with the student.
 
*Three copies of the wet-signed Voluntary/Mandatory Summer Internship Form must be signed. One copy will remain with the institution where the internship will be conducted (the employer), one copy will remain with the faculty, and one copy will remain with the student.
 
The areas that must be signed in triplicate on the summer internship forms are:
1-Department chair,
2-Dean's assistant,
3-The student themselves
4-The institution where the internship will be conducted (employer)
 
*The student must submit their documents to the department at least 15 days before the start date of the internship.
 
Note: In order to complete the mandatory summer internship, the student must have successfully completed the EBE305-Normal Birth and Postpartum Period course.
